Personal Care & Support
Dignified, respectful support with all aspects of personal care and daily living.
Our personal care service supports adults who need help with the essential activities of daily life — without compromising their dignity, independence, or sense of self.
Support is tailored to each individual and may include assistance with washing, bathing, dressing, grooming, oral hygiene, continence care, and meal preparation.
This service includes:
- Morning and evening routines
- Meal preparation and nutritional support
- Moving and handling
- Continence care
- End-of-life personal care (where applicable)
Community & Social Engagement
Helping adults stay connected to their communities and the activities that matter to them.
Isolation and loneliness can have a serious impact on health and wellbeing. Our community support service is designed to help adults stay active, connected, and engaged with the world around them.
Support workers accompany service users to appointments, social activities, day centres, places of worship, shops, and leisure activities — always at the individual's own pace and in line with their preferences.
This service includes:
- Accompanying to appointments (GP, hospital, etc.)
- Access to leisure and recreational activities
- Shopping and errands support
- Support with social relationships and friendships
- Attendance at day services or community groups
- Support with faith and cultural activities
Supported Living
Enabling adults to live independently in their own homes with the right level of support.
Supported living is about providing the right level of support for someone to live in their own home — whether that's a few hours a week or more intensive daily support.
Our team works closely with the individual, their family, and their social worker to design a support package that genuinely meets their needs and goals, with flexibility built in as those needs change over time.
This service includes:
- Flexible support hours to suit individual needs
- Life skills development (cooking, budgeting, household management)
- Support with tenancy responsibilities
- Co-produced support and care plans
- Regular review and outcome monitoring
Respite Care
Short-term support so that regular carers can take a well-deserved break.
Family carers do an extraordinary job — but everyone needs a break. Our respite care service provides short-term, planned care and support to give regular carers time to rest and recharge, with the reassurance that their loved one is in safe, caring hands.
Respite care can be arranged as a one-off period or on a regular scheduled basis.
This service includes:
- Planned short-term care covering the full care plan
- Detailed handover from family carers
- Regular updates to family during the respite period
- Emergency respite (subject to availability)
Outreach & Floating Support
Time-limited support to build skills, confidence, and independence.
Outreach and floating support is designed for adults who need some help to develop the skills and confidence to live more independently — but who don't require a permanent intensive package of support.
Support is usually time-limited and goal-focused, helping the individual work towards outcomes that matter to them.
This service includes:
- Goal-setting and outcome-focused planning
- Support with housing, benefits, and finances
- Health and wellbeing support
- Building daily living skills
- Connecting with community resources
Transitions Support
Supporting young adults (18+) as they move from children's services into adult care.
The transition from children's services to adult social care can be a challenging and confusing time for young people and their families. Our transitions support service helps young adults and their families navigate this process, ensuring continuity of care and a positive start to adult life.
We work closely with children's services, SEND teams, education providers, and adult social care to plan transitions well in advance and reduce disruption.
This service includes:
- Transition planning from age 17 (or as required)
- Liaison with children's services and adult social care
- Support during the transition period
- Education, employment, and training support
- Moving into supported living or independent accommodation
